Background Screening for Adults Affiliated with Junior
Teams |
| All adults affiliated with junior teams (coaches,
assistant coaches, club directors, tournament directors, trainers,
chaperones and volunteers) must pass the mandatory USAV Background
Screening. Background screenings performed by or for other
organizations cannot be substituted for this process. Background
screening is good for two seasons. |

Background Screening Policy
- USAV mandated policy,
including disqualifying offenses. |

Background Screening Consent and Waiver Form
- Apply online, then sign and submit the form from the Member Home
Page, or use this form. Background Screening does not begin
until the form and payment are received in the Region Office. |

Re-screening Memo - Club
Directors must request a re-screening of individuals if they become
aware of infractions that would disqualify the individual. |
